In accordance with the Royal Academy of Dance (RAD) Recruitment & Selection Policy, Criminal Record Checks & Recruitment of Ex-Offenders Policy and Safeguarding Policy & Procedures, candidates shortlisted for interview are required to complete this form if they have any criminal records.

Under the Exceptions Order to the Rehabilitation of Offenders Act 1974, the RAD is entitled to ask applicants for work with children to disclose any criminal records they have, including spent convictions.   Therefore applicants applying for such positions are required to include spent convictions.

In addition to the self-declaration form appointment to positions involving contact with children and vulnerable adults will be subject to a satisfactory Disclosure from the Disclosure and Barring Service (DBS), Disclosure Scotland or other equivalent authority.
